Where we are located . . .

Coastal Counseling Center is located in the
Lakeshore Medical Office Building
(Suite D) next to Cumberland Medical
Equipment.  The Lakeshore office complex
is directly across from the hospital's (old)
emergency room entrance.  Our business
hours are Monday through Friday from 9:00
until 5:00.  Evening hours may be available
and coordinated with each counselor.
FIRST TIME APPOINTMENTS:  The Intake Packet . . .

We have made our Intake Forms into fillable (point, click & type) documents that
open in Adobe Reader.  It is as easy as 1 - 2 - 3.

STEP ONE:  Choose the applicable intake packet:

            Adult Intake Packet        OR        Youth Intake Packet

  In Addition to the Intake Packets, please select the applicable reports:

1.  For YOUTH, Parents are required to complete the Parent Report

2.  For ADDICTIONS ASSESSMENTS, Please complete the Addictions Inventory

3.  For ANGER MANAGEMENT, Please complete the Anger Inventory

STEP TWO:    Point, click and type (using TAB or mouse).

STEP THREE:    Print or Save Instructions

  • Click PRINT.  You may print the forms and bring them with you . . . or

  • Click FILE, SAVE AS, PDF.  Please save as your first name (for example,
    dale).  For a second document, like the anger inventory, save as your first
    name and a number (for example, dale2).  Then email the form(s) to
    coastal@tds.net

The Red Ladies Are In Town!
In our effort to promote awareness of domestic and family violence, the Camden
County Family Violence Task Force is sponsoring the "Walk a Mile in her Shoes"
event.  The walk will be during the Mardi Gras parade on February 18, 2012.

We are inviting men who are prominent in our community to walk the parade
route wearing high-heel shoes.  All men and women are invited to register online
to walk in the parade to raise awareness of domestic and family violence.  All
proceeds from the registration will go to benefit Camden House and the Task
