by the Center as well as questionnaires for personal use.
Questionnaires

The link below will take you to Dr. Grohol's questionnaires (Psych Central).  
They are easy to complete and once you submit your answers, you receive
an immediate score and recommendations.  These questionnaires are not
intended to diagnose or treat related clinical problems or diagnoses.  They
are intended to assist you in clarifying symptoms that may be present and
may call for attention in counseling.  We recommend that you download the
form, complete it and bring it to your counselor for discussion.
